Overview
HeyForm is an open-source builder for forms, surveys, quizzes and polls. Forms start from templates or are generated with AI, and adapt to answers using conditional logic, answer piping and multiple endings. Responses can be reported on, downloaded, and pushed to tools such as Zapier, Google Sheets, Stripe and Slack.
